Dementia

If you are living with dementia, you are very likely to lose the ability to make decisions about your health in the later stages. As the future is uncertain, you could name someone close to you (called a 'key person') who, after talking to healthcare professionals and carers, can agree that you can no longer make your own decisions about your health. The person you choose will not be able to make formal decisions about your care or the treatment you receive, they will simply be part of the discussion with your healthcare professionals and carers.

Decide at what point your Advance Decision should be triggered

As dementia is a progressive illness, you may wish to indicate at which stage in your illness you would want your Advance Decision to be triggered. This helps those who will be caring for you to understand your wishes and will guide your carers to make decisions in your best interests.

Select any or all of the statements below if you wish these to be added to your Advance Statement. You can modify any of these statements. You may also add additional statements by selecting 'Add Statement' under 'Document Builder'. General guidance on Advance Statements can be found here.

If I am living with dementia and can't recognise people close to me such as family members, then I would hope my Advance Decision will come into effect.
