Neuro-degenerative disease - refuse treatment in crisis

Advance Decision for an acute complication

You may recover from your present crisis, such as respiratory failure or severe infection, and you may wish to receive every treatment possible. However, you may wish withdrawal of assisted breathing or wish to refuse some or all of the following treatments.

If you would like withdrawal or refusal of treatment in any or all of these scenarios, select the relevant options for the decisions to be added to your Advance Decision.

If you have difficulty breathing either because of aspiration into your lungs or developed a chest infection. In this circumstance:

  • You may wish withdrawal of assisted breathing.
  • You may refuse assisted breathing with a face mask, known as Continuous Positive Airway Pressure (CPAP).
  • You wish to be kept comfortable and have every treatment to keep you comfortable, for example being sedated, to ensure you do not suffer pain or discomfort.

If you have difficulty breathing either because of aspiration into your lungs or developed a chest infection. In this circumstance:

  • You may refuse the insertion of a tube in your windpipe (tracheostomy).
  • You wish to be kept comfortable and have every treatment to keep you comfortable, for example being sedated, to ensure you do not suffer pain or discomfort.

  • I do not want assisted breathing with a tracheostomy.

You may get a severe infection that, if not treated (with or without artificial ventilation) could result in your death. The most common life threatening infection is a lung infection (pneumonia). In this circumstance:

  • You can decide you do not want any treatment that may keep you alive. Refusing treatment for a serious infection may shorten your life.
  • You wish to be kept comfortable and have every treatment to keep you comfortable, for example being sedated, to ensure you do not suffer pain or discomfort.

  • If I get a severe infection that, if not treated (with or without artificial ventilation), could result in my death. Then I do not want treatment that may keep me alive.

If you stop breathing or your heart stops. In this circumstance:

  • You can decide you do not want to be resuscitated. Refusing Cardio-Pulmonary Resuscitation (CPR) will cause your life to end.

  • If I stop breathing or my heart stops, I do not want to be resuscitated.

 

If you are suffering from a neuro-degenerative disease, and are in a crisis from which you may recover, nevertheless you may wish withdrawal of these treatments.
This will apply if you are in coma or a minimally conscious state or have lost the ability to make decisions about your health.

If you are on any form of life-support (e.g. assisted ventilation, artificial feeding or hydration) this should be withdrawn.
You recognise that withdrawal of the above treatments would mean you would die. You wish to have every other treatment e.g. pain relief and / or sedation to keep you comfortable.

    Inability to communicate

  • If you are suffering from a neuro-degenerative disease, and if you have lost the ability to communicate about your health you may wish withdrawal or refusal of the following treatments:

  • If I can't swallow fluids or eat, I refuse any form of tube feeding or hydration.
