Dementia - considerations for your Advance Statement

Decide at what point your Advance Decision should be triggered

Your Advance Decision will not be used until you are no longer able to make decisions about your health and welfare.

Before making any choices, you should Register on the web site. If you have previously registered then please login. Register and login are seen in grey in the top right hand side of the screen.
By registering and / or logging in all your choices will be saved on the secure server so that you can return and see your previous choices. These can be altered at any time.

To allow you to state at which stage in the progress of dementia you would want your Advance Decision to be triggered, you can include in your Advance Statement which particular symptoms your health care professionals should take into account. These symptoms will guide the health care professionals to make decisions in your best interests.

Add your preferences and wishes to an Advance Statement

If I am living with dementia and can't recognise people close to me such as family members, then I would hope my Advance Decision will come into effect.

I wish my family to remember me as being active and able to engage in conversation. In my last years, I would prefer my children and grandchildren not to see me as a mere shadow of myself. I would hope my Advance Decision will come into effect if I am ill.

Care Home.
When my dementia gets to the stage that I don�t recognise family members or close friends and I am still physically active then, if my behaviour creates problems for those looking after me I would hope that a care home will be found for me.
